Population Health Analytics to Reduce Readmission Rates for Cardiac Care Patients
BUSINESS NEED: Chronic heart failure affects 6 million Americans today. More than 75% of health care costs are due to chronic conditions. Hospitals across the nation are focused on the triple aim of improving the patient experience, improving the health of the population and reducing the cost of healthcare. A large hospital asked for help in analyzing cardiac care patients outcome and measurably decrease readmission rates. CHALLENGE: The hospital was using Cerner for electronic medical records and Siemens for financial management. They had begun to build a separate SQL based data warehouse. However, they did not have the ability to complete correlation analysis of the dissimilar data in these two systems, nor the ability to complete an unstructured analysis of the physician notes fields, where critical patient information is often stored (e.g. ejection fraction is stored in notes). RESULTS: Stacy, as a supporting practice adviser of Cisco System, lead the design, architecture and solution to build the cardiac care dashboard. This included creating patient segmentation, analyzing patient and financial data, connecting to outside data pools (e.g. pharmacies), and completing Natural Language Processing and cluster analysis to pinpoint the highest risk patients. In knowing that older women, who have spend more than 5 days in the ICU are at a higher risk for readmission within 30 days of treatment for a cardiac issue (for instance), so that additional education and care can be taken prior to discharge. We expect to reduce readmission rates with this type of predictive analytics combined with preventative care by 2-4%.

Business Need: When M&A occurs, the greatest need is speed to delivery of the business case. We delivered this by bridging insights across disparate systems. Challenge: As the acquisition occurred, speed to value was paramount. We delivered integrated dashboards for financials and customer service to speed the merger to completion and deliver on the promised value.

Analytics to Improve Customer Service
BUSINESS NEED: A large customer service contact center, with razor slim margins and challenging customer service level agreements, needed the ability to manager to customer service levels, and therefore analyze internal all customer service data. Based upon the super-set of data, the company needed to pinpoint operational deficiencies and quickly resolve them to meet those difficult service level requirements. Both fines and perhaps even loss of customers was on the line. CHALLENGE: The customer service team was running in the dark, with no ability to analyze performance. There were many disparate systems where the core data was stored and the company had no single view of all of the data sets. Even within a single system, there was no ability for at a glance views of data, views over time, analysis of classes of data, or analysis by team. RESULTS: Because of our deep understanding of big data as well as being certified in Qlikview, we were in an excellent position to provide the ETL, via open source tools as well as the real-time analytics and dashboard. With real insights into the contact center performance, the company was able to exceed more than 20% of their very challenging service level requirements.
